IEP Bonus Evaluation Electronic Form

This is the newly REVISED IEP Bonus Evaluation Submission Page.  Here you will be able to submit your evaluation for bonus distribution. Please enter the data below under each heading and hit "Submit" after you have entered your answers.  Your answers will be e-mailed directly to Julie Susin.  She will then forward the info to the Board of Directors for review.  Please only submit one form per bonus period.

Below is a breakdown of your bonus:

Productivity = 60% of your bonus

Discretionary = 40% of your bonus (please enter data below)

Lectures = 1/2 point

This includes lectures at Providence Main/Park, Garden City, St. Joseph Mercy-Oakland, Oaklawn, EMS, ACLS, Fire Department, Nurses, etc.

Must give one lecture every six months.

Corporate Commitment = 5 points

This includes Scheduling, Task Force, Special Projects, Community Service, Board of Directors Meeting (50%), Other.

Charting, Business Meetings, and Mandatory IEP Meetings = potential -1 point for each (-3 total)

If notified more than three (3) times that your dictations are greater than 72 hours behind -- this results in loss of 1 point.  If notified again, the result will be -1 point.
